Cyclosporine Dosage in Patients with End-Stage Renal Disease on Hemodialysis
Introduction to Cyclosporine Dosage
- The American Society of Nephrology recommends a daily dose of 120-160 mg (3-4 mg/kg/day) for a 40kg adult patient on hemodialysis, divided into two doses, administered after hemodialysis on dialysis days 1, 2, 3
Dosage Administration
- The dose should be administered in two divided doses (60-80 mg twice daily) 1, 2, 3
- The dose depends on the underlying disease (nephrotic syndrome, transplantation, etc.) 1
Timing of Administration
- The medication should always be administered after hemodialysis on dialysis days to avoid any loss of the drug 4, 5
- This also facilitates directly observed therapy (DOT) 4
Target Levels and Monitoring
- The target trough level (C0) is 125-175 ng/mL (non-toxic range) 1, 2, 3
- The target level 2 hours after dosing (C2) is 400-600 ng/mL 1, 2
- Monitoring of levels is essential to ensure adequate absorption without excessive accumulation 4
Dose Reduction
- After achieving the therapeutic goal, the dose should be gradually reduced to the smallest effective dose 1, 2, 3
- For complete remission, the dose should be reduced by 0.5 mg/kg/month 2, 3
- For partial remission, the full dose should be continued for 1-2 years 2, 3
Important Warnings
- Cyclosporine can cause nephrotoxicity even in patients with pre-existing renal disease 1, 2, 3
- Renal function (GFR) should be closely monitored, with a target of maintaining ≥70% of the pre-treatment value 1, 2
- Long-term studies show that after initial reduction, GFR stabilizes with chronic cyclosporine use 1, 2
- Blood pressure should be monitored, with a target of <120/75 mmHg 1, 3
- Hypertension is a common adverse effect and may be exacerbated by cyclosporine 1, 3
- Hyperkalemia and hypertrichosis are common adverse effects 1
- Electrolytes, particularly potassium and magnesium, should be monitored 1
