Anesthesia Management in Systemic Lupus Erythematosus (SLE) Patients

Preoperative Evaluation

  • The American College of Rheumatology recommends evaluating kidney function, including serum creatinine, proteinuria, and urine sediment, as 40% of SLE patients develop lupus nephritis 1
  • The American College of Rheumatology suggests screening for antiphospholipid antibodies, which are crucial in determining the risk of thrombosis 1
  • Patients with SLE and persistent proteinuria, GFR <60 mL/min, and chronic glucocorticoid use have a higher risk of cardiovascular events 2

Preoperative Optimization

  • The American College of Rheumatology recommends continuing hydroxychloroquine in all patients (≤5 mg/kg actual body weight) to reduce disease activity and mortality 3, 2
  • The American College of Rheumatology suggests evaluating and adjusting glucocorticoid doses, targeting <7.5 mg/day prednisone equivalent for elective surgery 3, 4
  • For high-risk surgeries with cardiopulmonary or severe neuropsychiatric involvement, consider optimization with immunosuppressants (mycophenolate mofetil or azathioprine) 3, 2

Intraoperative Management

  • The American Society of Anesthesiologists recommends general anesthesia with endotracheal intubation and controlled ventilation as a safe and recommended choice for most procedures 5
  • The American Society of Anesthesiologists suggests avoiding the use of epinephrine in local anesthesia, as it can trigger vasospasm and thrombosis 5
  • Continuous monitoring of blood pressure, pulse oximetry, and temperature is crucial to prevent hypotension, hypoxia, and hypothermia 7, 6

Postoperative Management

  • The American Society of Anesthesiologists recommends structured postoperative oxygen therapy to maintain SpO2 above baseline or 96% (whichever is higher) for 24 hours 7, 6
  • The American Society of Anesthesiologists suggests early mobilization and physiotherapy to prevent thrombosis and promote recovery 7, 6
  • The American Society of Anesthesiologists recommends thromboprophylaxis for all peri- and post-pubertal patients, as SLE patients have an increased risk of deep vein thrombosis 7, 6

Special Considerations for Cardiac Surgery

  • The American Heart Association recommends that cardiac surgery in SLE patients is feasible and safe with proper protocols 5
  • The American Heart Association suggests using cardiopulmonary bypass in normothermic or hypothermic conditions, and hemofiltration is often used in combination with these procedures 5

Pain Management

  • The American Society of Anesthesiologists recommends using validated pain assessment scales and reassessing regularly to manage postoperative pain effectively 7, 6
  • The American Society of Anesthesiologists suggests notifying the acute pain service if patients have a history of opioid tolerance 7, 6

Infection Prevention

  • The Centers for Disease Control and Prevention recommend providing prophylactic antibiotics according to surgical protocols 7
  • The Centers for Disease Control and Prevention suggest monitoring signs of respiratory and wound infections postoperatively 7
