Nitroglycerin Infusion Protocol for Acute Pulmonary Edema

Introduction to Nitroglycerin Therapy

  • The European Society of Cardiology recommends starting IV nitroglycerin at 10-20 mcg/min using non-PVC tubing and titrating upward by 5-10 mcg/min every 3-5 minutes until symptoms improve, maintaining systolic blood pressure above 90 mmHg 1, 2
  • Nitroglycerin is recommended when systolic blood pressure (SBP) is >110 mmHg and relieves pulmonary congestion by reducing preload and afterload without compromising stroke volume, as suggested by the European Heart Journal 1, 2

Blood Pressure Requirements and Titration

  • Use with extreme caution if SBP is 90-110 mmHg, with more frequent blood pressure monitoring and slower titration, as recommended by the European Heart Journal 1, 2
  • Increase by 5-10 mcg/min every 3-5 minutes based on blood pressure response and symptom control, according to the Journal of the American College of Cardiology 1, 2, 3, 4

Monitoring and Contraindications

  • Measure blood pressure every 3-5 minutes during active titration to avoid precipitous drops in SBP, as suggested by the European Heart Journal 1, 2
  • Consider arterial line placement for doses >50-100 mcg/min or in patients with borderline blood pressure to facilitate precise titration, according to the Journal of the American College of Cardiology and the European Heart Journal 1, 2, 3, 4
  • Phosphodiesterase inhibitor use is an absolute contraindication within 24 hours of sildenafil or 48 hours of tadalafil due to risk of fatal hypotension, as stated by the Journal of the American College of Cardiology 5

Adverse Effects and Management

  • Headache is frequently reported but rarely requires discontinuation, according to the European Heart Journal 1, 2
  • Tachyphylaxis develops after 24-48 hours of continuous infusion, necessitating incremental dose increases or transition to alternative therapy, as recommended by the European Heart Journal and the Journal of the American College of Cardiology 1, 2, 3, 4

Integration with Comprehensive Therapy

  • Combine with furosemide for diuresis, though vasodilators are now prioritized over diuretics in acute management, as suggested by the American College of Cardiology 5
  • Apply non-invasive positive pressure ventilation (BiPAP or CPAP) simultaneously to improve oxygenation and reduce work of breathing, according to the American College of Cardiology 5
  • Administer supplemental oxygen to maintain saturation >90%, as recommended by the American College of Cardiology 5
  • Consider morphine sulfate 2-5 mg IV for anxiolysis and additional venodilation, though use has become more selective, according to the American College of Cardiology 5
  • Beta-blockers may be combined in appropriate patients to reduce risk of reflex tachycardia, as suggested by the Journal of the American College of Cardiology 3, 4
