Ulcerative Colitis in Toddlers

Key Clinical Considerations

  • The American Gastroenterological Association notes that toddlers can develop ulcerative colitis, with approximately 10-15% of all UC patients diagnosed before age 18 years, and cases documented in children as young as 2.5 years of age 1
  • Young children under 10 years show significantly less crypt architectural distortion and inflammation compared to adolescents or adults, with atypical histological patterns 2
  • The European Society for Pediatric Gastroenterology, Hepatology and Nutrition recommends that upper GI evaluation is essential, as up to 75% of children with UC show upper GI inflammation (esophagitis, gastritis, or duodenitis) 3
  • The North American Society for Pediatric Gastroenterology, Hepatology and Nutrition suggests considering very early onset IBD in children diagnosed by age 6, who may have underlying immune deficiencies requiring special investigation 4

Diagnostic Approach

  • The American College of Gastroenterology recommends that the gold standard for diagnosis remains endoscopic evaluation with mucosal biopsies from both upper and lower GI tracts 1
  • Multiple biopsies are required, as adequate tissue sampling is crucial since histologic features may be subtle or patchy 3

Differential Diagnosis

  • Infectious colitis (particularly Campylobacter and Yersinia) can mimic UC in toddlers, according to the Centers for Disease Control and Prevention 2
  • Crohn's disease is also a consideration in the differential diagnosis of UC in toddlers, as noted by the Crohn's and Colitis Foundation 3
