Management of Exocrine Pancreatic Insufficiency

Diagnosis and Interpretation

  • Fecal elastase levels >200 μg/g indicate normal pancreatic function, while levels <100 μg/g are consistent with exocrine pancreatic insufficiency (EPI) 1
  • The American Gastroenterological Association recommends that fecal elastase testing be performed on semi-solid or solid stool samples for accuracy 1

Treatment and Management

  • The American College of Gastroenterology recommends initiating pancreatic enzyme replacement therapy (PERT) immediately with 500 units of lipase per kg per meal and 250 units per kg per snack for patients with EPI 1
  • PERT should be taken during meals, not before or after, with a maximum dose of 2,500 units lipase/kg per meal or 10,000 units/kg/day 1
  • A protein-rich diet should be implemented, and very low-fat diets should be avoided, with moderate fat intake being appropriate with adequate enzyme replacement 1
  • Fat-soluble vitamins (A, D, E, K) and micronutrients should be supplemented 1

Monitoring and Dose Titration

  • Clinical parameters to assess treatment response include reduction in steatorrhea, improvement in abdominal pain, bloating, and diarrhea, weight gain, and increased muscle mass 1
  • Laboratory monitoring should include fat-soluble vitamin levels, annual glucose and HbA1c, and micronutrient status annually 1
  • If inadequate response to initial PERT dosing, the dose should be increased incrementally up to a maximum of 2,500 units/kg per meal 1

Adverse Effects and Warnings

  • Fibrosing colonopathy is a rare but serious complication, especially in children with cystic fibrosis on high doses, and patients should be monitored for abdominal pain, bloating, constipation, nausea, and vomiting 1
  • Other adverse effects to monitor include hyperuricemia, hypersensitivity reactions, and mouth irritation if capsules are not swallowed completely 1
  • The European Society for Pediatric Gastroenterology, Hepatology, and Nutrition recommends adhering strictly to maximum dosing limits to minimize the risk of adverse effects 4
