High IgE Levels in Pregnancy: Causes and Investigations

Primary Causes and Investigations

  • Elevated IgE levels during pregnancy are most commonly caused by pre-existing atopic conditions, such as allergic rhinitis, asthma, atopic dermatitis, and food allergies, and investigations should focus on identifying these allergic triggers through specific IgE testing or skin prick testing, complete blood count with differential for eosinophilia, and stool examination if parasitic infection is suspected, as recommended by the American College of Allergy, Asthma, and Immunology 1
  • Parasitic infections must be considered, particularly in endemic areas or with travel history, and can be identified through stool examination 1
  • Exposure to environmental allergens, such as fungi, dust mites, and pollens, can trigger IgE elevation, and avoidance of these allergens is recommended 2
  • Tobacco smoke exposure and air pollutants are associated with increased IgE levels, and avoidance of these exposures is recommended 2
  • Complete blood count with differential to assess for eosinophilia is recommended as an initial laboratory workup 1
  • Specific IgE testing or skin prick testing to identify suspected allergens has a negative predictive value of >95% and is recommended as an initial laboratory workup 1
  • Stool examination is recommended if parasitic infection is suspected based on risk factors 1
  • Women on maintenance allergy immunotherapy can safely continue during pregnancy without dose increases, according to the American Academy of Allergy, Asthma, and Immunology 3
  • Do not initiate allergy immunotherapy during pregnancy, except in life-threatening cases of Hymenoptera anaphylaxis, as recommended by the American College of Allergy, Asthma, and Immunology 3
  • Focus on allergen avoidance for documented IgE-mediated allergies, and use inhaled corticosteroids for persistent allergic asthma, and antihistamines for allergic rhinitis and urticaria, as recommended by the American College of Allergy, Asthma, and Immunology 1
  • Strict adherence to safety protocols is critical, as systemic allergic reactions could cause spontaneous abortion, premature labor, or fetal hypoxia, and is recommended by the American Academy of Allergy, Asthma, and Immunology 4, 3
