Neutrophilic Leukocytosis with Left Shift and Toxic Granules: Diagnosis and Management

Diagnostic Interpretation

  • An absolute band count ≥1,500 cells/mm³ has the highest diagnostic accuracy (likelihood ratio 14.5) for documented bacterial infection 1
  • Band neutrophil percentage ≥16% of total WBCs carries a likelihood ratio of 4.7 for bacterial infection, even when total WBC count is normal 1, 2
  • Manual differential count is essential—automated analyzers cannot reliably assess band forms and immature neutrophils 1

Clinical Assessment Algorithm

  • Fever or hypothermia (temperature >38°C or <36°C) is a sign of severe infection or sepsis 3, 4
  • Hypotension (<90 mmHg systolic or decrease >40 mmHg from baseline) is a sign of severe infection or sepsis 3, 4
  • Tachycardia, tachypnea, altered mental status are signs of severe infection or sepsis 3, 4
  • Hyperlactatemia (>3 mmol/L), oliguria (<30 ml/h or <0.5 ml/kg/h) are signs of severe infection or sepsis 3, 4
  • Respiratory tract (cough, dyspnea, chest pain) is a potential infection source 2
  • Urinary tract (dysuria, flank pain, frequency) is a potential infection source 2, 5
  • Skin/soft tissue (erythema, warmth, purulent drainage) is a potential infection source 2

Management Approach

  • Initiate broad-spectrum empiric antibiotics within 1 hour of recognition of severe sepsis or septic shock 6, 3
  • Aggressive fluid resuscitation for hypotension in severe sepsis or septic shock 3, 4
  • Source control measures (drainage of abscesses, removal of infected catheters) are essential in severe sepsis or septic shock 6
  • Vasopressor support if hypotension persists despite fluid resuscitation in severe sepsis or septic shock 3, 4
  • Complete diagnostic workup before antibiotics if patient is hemodynamically stable 2, 5
  • Initiate targeted antibiotic therapy based on suspected source and local resistance patterns 2
  • Close monitoring for clinical deterioration is essential 2, 5

Important Caveats and Pitfalls

  • Do not rely on automated analyzer flags alone—manual differential is mandatory for accurate assessment 1
  • Do not ignore left shift when total WBC is normal—this combination still indicates significant bacterial infection 1, 2
  • Do not delay antibiotics in severe sepsis/septic shock while awaiting culture results 6, 3
  • Do not treat asymptomatic patients with antibiotics based solely on laboratory findings without clinical correlation 2, 5
