Management of Subdural Hematoma in Pediatric Patients

Initial Assessment and Imaging

  • The American College of Radiology recommends performing immediate non-contrast CT scan of the head to evaluate hematoma characteristics, including maximal thickness, degree of midline shift, and presence of mass effect 2, 1
  • The initial assessment should include documentation of the Glasgow Coma Scale (GCS) score, pupillary examination, focal neurological deficits, and level of consciousness, as recommended by Praxis Medical Insights 1
  • Funduscopic examination is mandatory to check for retinal and preretinal hemorrhages, which are characteristic findings in infantile subdural hematoma, according to the Journal of the American College of Radiology 2, 3

Surgical Indications

  • The American College of Radiology indicates that immediate surgical evacuation is recommended for symptomatic subdural hematoma with significant mass effect, neurological deterioration, decreased level of consciousness, or increased intracranial pressure refractory to medical management, as suggested by Praxis Medical Insights 1
  • Surgical evacuation is the preferred approach for acute subdural hematomas requiring surgery, as stated by Praxis Medical Insights 1

Conservative Management Protocol

  • Praxis Medical Insights suggests that conservative management with close monitoring is appropriate for stable patients without significant neurological deficits, including regular neurological assessments, maintenance of euvolemia, and serial imaging 1

Additional Considerations for Pediatric Patients

  • The Journal of the American College of Radiology recommends considering child abuse evaluation, including skeletal survey, MRI of the brain, and MRI of cervical spine, in pediatric patients with subdural hematoma 2, 3
  • The American College of Radiology suggests that early recognition and suitable treatment may improve outcome and prevent progression to chronic subdural hematoma, as stated by Praxis Medical Insights 1

Outcome Expectations

  • Praxis Medical Insights states that delaying surgical intervention when neurological deterioration occurs leads to poorer outcomes, emphasizing the importance of prompt intervention 1
