Guideline for Evaluation of Mild Pancytopenia with Normocytic Anemia

Definition and Initial Classification

  • Anemia confirmation – Hemoglobin < 12 g/dL in adult women meets WHO criteria for anemia, confirming the presence of anemia in this patient. 1
  • Normocytic anemia assessment – Calculated mean corpuscular volume (MCV) ≈ 95 fL falls within the normal range (80–100 fL), indicating a normocytic pattern. 1

Etiology of Pancytopenia

  • Multilineage cytopenia warrants hematology evaluation – When two or more blood cell lines are reduced, a systematic work‑up and likely hematology consultation are recommended. 2
  • Potential reversible and irreversible causes –

    • Bone‑marrow failure syndromes (e.g., aplastic anemia, myelodysplastic syndrome)
    • Nutritional deficiencies (vitamin B12, folate, iron)
    • Autoimmune disorders (e.g., systemic lupus erythematosus)
    • Chronic infections (HIV, hepatitis)
    • Medication‑induced marrow suppression (chemotherapy, immunosuppressants, certain antibiotics)
    • Hypersplenism
    • Bone‑marrow infiltration (leukemia, lymphoma, metastatic disease)
  • Anemia of chronic kidney disease (CKD) – CKD‑related anemia is typically normochromic and normocytic, reflecting erythropoietin deficiency. 2

Core Diagnostic Work‑up

Laboratory Tests

Test Rationale (Evidence)
Peripheral blood smear Essential to detect dysplastic cells, blasts, or other morphological clues to marrow pathology. [2]
Reticulocyte count Low reticulocyte → inadequate marrow response (nutritional deficiency, marrow failure, CKD, inflammation). High reticulocyte → hemolysis or acute blood loss. [2]
Complete iron studies (serum ferritin, TSAT, serum iron, TIBC) Determines iron deficiency versus anemia of chronic disease. [2]
Vitamin B12 and folate levels Excludes macrocytic anemias due to cobalamin or folate deficiency. [2]
Comprehensive metabolic panel Evaluates renal function (BUN, creatinine) to identify CKD‑related anemia. [2]
C‑reactive protein (CRP) or erythrocyte sedimentation rate (ESR) Elevated inflammatory markers support anemia of chronic disease and help interpret ferritin results. [1]
Medication review Identifies drugs that may cause cytopenias (e.g., NSAIDs, anticoagulants, chemotherapy, sulfonamides). [1]
Menstrual and bleeding history Heavy menstrual bleeding is a leading cause of iron deficiency in pre‑menopausal women. [1]

Interpretation of Iron Studies (Praxis thresholds)

  • Ferritin < 30 µg/L (without inflammation) → confirms iron deficiency. 1
  • Ferritin 30–100 µg/L with elevated CRP → possible iron deficiency masked by inflammation. 1
  • Transferrin saturation (TSAT) < 15–16 % → indicates iron deficiency. 1
  • Low serum iron + elevated TIBC → classic iron‑deficiency pattern. 1
  • Ferritin > 100 µg/L + TSAT < 20 % → suggests anemia of chronic disease. 1

Note: Ferritin is an acute‑phase reactant; inflammation, infection, or malignancy can falsely elevate it, so always interpret alongside CRP. 3

Referral Recommendations

  • Urgent hematology referral – Indicated if peripheral smear shows blasts or dysplastic cells, if pancytopenia persists without a reversible cause, if reticulocyte count remains inappropriately low after nutritional repletion, or if no improvement after 3 months of targeted therapy. 2

  • Gastroenterology referral – Required when iron deficiency is confirmed without an obvious source (e.g., heavy menses, dietary insufficiency), when gastrointestinal symptoms are present, when oral iron fails to correct anemia, or when occult GI bleeding is suspected. 2

  • Nephrology referral – Considered when elevated creatinine or reduced eGFR indicates CKD as the underlying driver of normocytic anemia. 2
