Treatment of Salmonella Bacteremia

Initial Empiric Therapy

  • The American College of Physicians recommends combination therapy with ceftriaxone 2g IV once daily plus ciprofloxacin 500-750mg twice daily for bacteremia to prevent initial treatment failure before susceptibility results are available 1, 2
  • This dual approach covers potential fluoroquinolone resistance while awaiting culture data 1
  • Once susceptibilities are known, de-escalate to single-agent therapy 1

Definitive Monotherapy (After Susceptibility Testing)

  • The Centers for Disease Control and Prevention recommends ciprofloxacin 500-750mg twice daily as the preferred fluoroquinolone for susceptible organisms 3, 4
  • Ceftriaxone 2g IV once daily is an effective alternative, especially for fluoroquinolone-resistant strains 1
  • TMP-SMZ is an alternative if the organism is susceptible 1

Treatment Duration

  • The Infectious Diseases Society of America recommends a minimum treatment duration of 14 days for immunocompetent patients 3, 2
  • Immunocompromised patients require longer treatment courses, typically 2-6 weeks 2
  • HIV-infected patients with bacteremia require long-term suppressive therapy to prevent recurrence 3, 4, 2

Special Populations

HIV/Immunocompromised Patients

  • The CDC recommends always treating bacteremia in HIV-infected patients due to the high risk of dissemination 3, 5, 2
  • Long-term suppressive therapy with ciprofloxacin is required after initial treatment to prevent recurrence 3, 4

Pregnant Women

  • The American College of Obstetricians and Gynecologists recommends avoiding fluoroquinolones entirely 3, 5, 4, 2
  • Ceftriaxone, cefotaxime, ampicillin, or TMP-SMZ can be used based on susceptibility 5, 2

Children

  • The American Academy of Pediatrics recommends using fluoroquinolones with extreme caution and only if no alternatives exist 3, 5, 4, 2
  • Preferred agents include TMP-SMZ, ceftriaxone, cefotaxime, or ampicillin 5, 2

Critical Considerations

  • The World Health Organization notes that fluoroquinolone resistance is increasing, particularly in certain serotypes, justifying the initial combination therapy approach until susceptibilities are confirmed 1

Monitoring and Follow-up

  • The Infectious Diseases Society of America recommends repeat blood cultures if bacteremia persists to identify undrained foci 6
  • Transition to oral therapy (ciprofloxacin 500mg twice daily) is appropriate once clinically improved and bacteremia has cleared 2
