Elevated Vitamin B12 Levels and Associated Conditions

Clinical Significance and Diagnostic Approach

  • In patients with myeloid neoplasms with eosinophilia, elevated serum vitamin B12 is commonly observed, particularly in those with PDGFRA fusion genes, according to the National Comprehensive Cancer Network 1
  • A complete blood count with differential should be performed to assess for other hematologic abnormalities in patients with elevated vitamin B12 levels, as recommended by the National Comprehensive Cancer Network 1
  • A comprehensive metabolic panel with liver function tests should be conducted to evaluate for liver disease in patients with elevated vitamin B12 levels, as suggested by the National Comprehensive Cancer Network 1
  • Serum tryptase levels should be measured, as they are often elevated alongside B12 in myeloproliferative disorders, according to the National Comprehensive Cancer Network 1
  • For suspected hematologic malignancy, bone marrow aspirate and biopsy with immunohistochemistry should be performed, as recommended by the National Comprehensive Cancer Network 1
  • Cytogenetic and molecular testing for TK fusion gene rearrangements should be conducted in patients with suspected hematologic malignancy and elevated vitamin B12 levels, as suggested by the National Comprehensive Cancer Network 1

Monitoring and Management

  • If initial workup is negative but vitamin B12 remains persistently elevated, periodic monitoring with complete blood count and liver function tests should be considered, as recommended by the National Comprehensive Cancer Network 1
