Prochlorperazine Safety in CKD
Monitoring Requirements
- The Kidney International guideline recommends checking renal function (serum creatinine and eGFR) to detect further deterioration within 2-4 weeks of initiation or dose changes in patients with CKD 1
- Monitoring of mental status and sedation level is necessary, as uremic patients are more sensitive to CNS effects, according to Kidney International 2
Dose Adjustment and Management
- Discontinue or further reduce the dose if serum creatinine rises by more than 30% from baseline, as recommended by Kidney International 1
- Discontinue or further reduce the dose if excessive sedation or confusion develops, as recommended by Kidney International 1
- Discontinue or further reduce the dose if extrapyramidal symptoms emerge or worsen, as recommended by Kidney International 1
Medication Interactions
- The Kidney International guideline advises avoiding combination of prochlorperazine with other nephrotoxic medications, such as NSAIDs, ACE inhibitors, or ARBs, in patients with CKD 2
- The Kidney International guideline recommends careful monitoring when combining prochlorperazine with ACE inhibitors or ARBs in patients with CKD 1
