Methotrexate with Prednisone for Rheumatoid Arthritis
Initial Treatment Regimen
- The American College of Rheumatology recommends starting oral methotrexate at 15 mg weekly and escalating to 25-30 mg weekly within 4-6 weeks, combined with short-term low-dose prednisone (5-10 mg daily) tapered over 2-4 months, which provides superior disease control, slows radiographic progression, and achieves remission in 40-50% of patients 1, 2, 3
Methotrexate Dosing Strategy
- The optimal therapeutic dose of methotrexate in Western populations is 20-25 mg per week, with maximum effect requiring 4-6 months of therapy 6, 7
- The European League Against Rheumatism suggests starting with oral methotrexate 15 mg weekly rather than lower doses or subcutaneous administration, and escalating by 5 mg every month to reach 25-30 mg weekly within 4-6 weeks 1, 2, 3, 5, 6
Folic Acid Supplementation
- Folic acid supplementation should always be prescribed to reduce gastrointestinal and other adverse effects, with the dose increased if tolerability issues arise 1, 2, 3, 5, 6, 8
Prednisone Dosing Strategy
- Short-term glucocorticoids (<3 months) are conditionally recommended over no glucocorticoids, with the goal of tapering to 5 mg daily by week 8 and continuing to taper over 2-4 months total 3
Treatment Monitoring and Adjustment
- The treatment target is remission or low disease activity within 6 months, with response assessed at 3 months and treatment modified if no improvement is seen 1, 2, 6, 7
- If there is an inadequate response on oral methotrexate, the first step is to switch to subcutaneous methotrexate before adding other DMARDs 1, 2, 3
Safety Monitoring
- Methotrexate should be held if serum creatinine increases by 50%, transaminases >2× upper limit of normal, or mucositis is present, and pneumocystis prophylaxis should be considered if prednisone ≥20 mg daily for ≥4 weeks 4
- Screening for hepatitis B/C and latent tuberculosis should be performed before starting therapy 4
