Management of Respiratory Compromise in Children

Immediate Oxygen Management

  • The American Academy of Pediatrics recommends starting high-flow oxygen via face mask immediately to maintain oxygen saturation >92% in children with hypoxemia, as levels below 92% are associated with major adverse events and require urgent correction 1, 2
  • Continuous pulse oximetry monitoring should be maintained throughout treatment to ensure oxygen saturation remains within the target range of 92-96% 1, 2

Bronchodilator Therapy

  • The American College of Chest Physicians recommends administering nebulized salbutamol 5 mg via oxygen-driven nebulizer every 20 minutes for 3 doses initially, as this is effective in severe respiratory compromise with hypoxemia 1, 2, 3
  • Ipratropium 100 mcg should be added to each nebulization immediately and continued every 6 hours, as the combination is specifically indicated when initial beta-agonist treatment fails or in severe exacerbations 1, 2, 3

Systemic Corticosteroids

  • The American Academy of Pediatrics recommends giving oral prednisolone 1-2 mg/kg (maximum 60 mg) immediately as a single dose, as systemic corticosteroids are essential in the treatment of severe asthma and pneumonia 1, 3
  • Oral route is preferred if the child can swallow and is not vomiting, as there is no advantage to IV administration when gastrointestinal transit is normal 1

Monitoring and Reassessment

  • The American Heart Association recommends repeat clinical assessment 15-30 minutes after starting treatment to evaluate response, and monitoring respiratory rate, work of breathing, ability to speak/feed, heart rate, and oxygen saturation continuously 1, 2, 3
  • Brief spot-checks of oxygenation are insufficient - continuous monitoring is required 4

Hospital Admission Criteria

  • The American Academy of Pediatrics recommends hospital admission for children who meet multiple criteria, including SpO2 88% (hypoxemia), pneumonia with wheezing, influenza infection, and likely persistent features of severe asthma after initial treatment 1, 2

Common Pitfalls to Avoid

  • The American College of Chest Physicians recommends not delaying systemic corticosteroids while continuing repeated albuterol doses alone - corticosteroids must be given immediately upon recognition of severe asthma 1
