Methylphenidate Dosage and Administration Guidelines

Age-Specific Starting Doses

  • The American Academy of Child and Adolescent Psychiatry recommends starting with 5 mg twice daily, administered in the morning and at lunch, for children 6 years and older, with dose increases of 5-10 mg weekly based on symptom control and side effects 1
  • For children weighing less than 25 kg, single doses should not exceed 15 mg 1
  • The American Academy of Pediatrics suggests considering methylphenidate only after behavioral interventions have failed and there is moderate-to-severe dysfunction in preschool children (4-5 years), starting with lower doses (typically 2.5 mg) due to slower metabolism in this age group 2, 3

Titration Protocol

  • The American Academy of Child and Adolescent Psychiatry recommends increasing the dose by 5-10 mg weekly for methylphenidate, using a consistent titration schedule to achieve optimal symptom control 1
  • Continue titration until maximum symptom control is achieved without adverse effects 4

Formulation Considerations

  • Immediate-release methylphenidate has a peak plasma concentration 1-3 hours after administration and an average half-life of 2 hours, requiring multiple daily dosing (typically 2-3 times daily) 1, 4
  • Extended-release formulations provide longer duration of action (8+ hours) and may be more convenient for school-aged children 4

Monitoring During Initiation

  • Assess response using standardized ADHD rating scales and reports from parents and teachers, and monitor for common side effects, particularly during the first few days of treatment 1, 4

Special Considerations

  • For adolescents, consider the risk of substance use and diversion before initiating treatment, and longer-acting or late-afternoon, short-acting medications may be helpful for adolescents who drive 2, 3
