Management of Diabetic Foot Ulcers in Elderly Patients with Limited Mobility

Frequency of Dressing Changes

  • The International Journal of Infectious Diseases recommends changing transparent dressings every 7 days at most, and gauze dressings every 2 days, with careful site care using chlorhexidine-based antiseptic 1, 2
  • The American Diabetes Association and other guideline societies suggest that dressings should be changed immediately if they become soiled, loose, or wet, or if there is significant bleeding, drainage, or signs of infection 1, 2, 3, 6

Protocol for Dressing Changes

  • During the first week after wound occurrence, clean the wound with sterile saline solution, dry the area completely to prevent maceration, and monitor daily for signs of infection or other complications 3, 6
  • After the first week, cleaning with potable water and mild soap is sufficient, and continue to dry the area completely before applying a new dressing 3, 6

Recommendations for Diabetic Foot Ulcers

  • The International Working Group on the Diabetic Foot (IWGDF) recommends using basic dressings that absorb exudate and maintain a moist healing environment, and avoiding antimicrobial or antiseptic topical agents, collagen or alginate dressings, honey or bee-related products, topical phenytoin, and herbal preparations 4, 5
  • Consider using dressings impregnated with sucrose octasulfate for non-infected neuroischemic ulcers that have not improved after 2 weeks of optimal standard care, including proper offloading 4, 5

Important Warnings

  • Avoid occlusive dressings, which can promote excessive moisture and lead to skin maceration 3, 6
  • In elderly diabetic patients with limited mobility, pay particular attention to complications that could lead to functional decline 7
  • Debridement should be determined by clinical need, not on a fixed schedule 4
