Orexin Inhibitors and Escitalopram in Patients with Depression and Insomnia

Safety Profile of the Combination

  • The American Academy of Child and Adolescent Psychiatry suggests that escitalopram has a lower propensity for drug interactions due to its minimal effect on CYP450 isoenzymes compared to other SSRIs 1, 2, 3

Precautions and Monitoring

  • The American Academy of Child and Adolescent Psychiatry recommends caution when combining two or more serotonergic drugs, including starting the second drug at a low dose and increasing slowly, and monitoring for symptoms of serotonin syndrome 1, 2, 3
  • The FDA warns that orexin antagonists like suvorexant may cause cognitive and behavioral changes, such as amnesia, anxiety, hallucinations, and complex behaviors like sleep driving 4

Clinical Approach to Special Populations

  • The American Heart Association, as published in Circulation, recommends that sleep disturbances in patients with cardiovascular disease should first be addressed with cognitive behavioral therapy for insomnia before medication 5, 6, 7
  • The American Academy of Child and Adolescent Psychiatry notes that escitalopram may cause QT prolongation at higher doses, so careful monitoring is advised when combining with other medications 1, 2, 3
  • Patients with cardiovascular disease may benefit from the combination of escitalopram and an orexin antagonist, as SSRIs are generally well-studied and safe in coronary heart disease and heart failure 5, 6, 7
