Exercise Program for Hip Conditions
Core Exercise Program Structure
- The American College of Rheumatology recommends a supervised progressive resistance exercise program targeting hip rotator muscles for at least 3 months with a minimum of 12 supervised sessions, emphasizing gluteus medius strengthening and hip motor control exercises 1
- Progressive strengthening exercises focusing on hip muscles, particularly the gluteus medius, form the foundation of rehabilitation 1
- Exercise parameters should be specified explicitly, including magnitude of load, number of repetitions and sets, duration of contractile component, time under tension, rest intervals between repetitions and sessions, and range of motion 1
- Hip and pelvis motor control exercises in single-leg support positions should be incorporated 1
Supervision and Delivery
- Supervised exercise programs are more effective than home-based programs alone, ideally delivered by physical therapists in class settings 2, 3, 4
- A minimum of 12 supervised sessions over at least 3 months duration is recommended 1
- Programs are enhanced when combined with self-efficacy interventions and weight loss programs (if overweight) 2, 3
Exercise Types and Progression
- Dynamic resistance exercises with progression over time are preferred over static isometric exercises 5
- Moderate to vigorous intensity (60-80% of one repetition maximum) for 8-12 repetitions is recommended 5
- Progressive strength training involving major muscle groups at least 2 days per week is recommended 5
- Moderate-intensity aerobic training for at least 30 minutes per day, up to 60 minutes for greater benefit, is recommended 5
Additional Exercise Modalities
- Tai chi is strongly recommended for hip osteoarthritis, combining meditation, slow movements, breathing, and relaxation 3, 4
- Balance exercises are conditionally recommended to improve body position control and stability 3, 6
- Neuromuscular training with dynamic maneuvers of increased complexity addresses muscle weakness and sensorimotor control 6
Adjunctive Interventions
- Weight loss counseling for overweight/obese patients, targeting ≥5% body weight reduction with progressive benefits up to >20% loss, is strongly recommended 2, 6, 7
- Self-management programs combining skill-building, education, joint protection, and fitness goals are strongly recommended 2, 3, 4
- Manual therapy combined with supervised exercise (not manual therapy alone) is strongly recommended 7, 8
Critical Implementation Points
- Active physiotherapeutic interventions (supervised exercise) should be prioritized over passive interventions like massage, ultrasound, or heat 1
- Specific, detailed exercise prescriptions rather than general encouragement to exercise are recommended 2, 3, 6
- Patient preferences and access considerations should be taken into account, as these determine adherence 6
