Hospice Referral and Care

Insurance Coverage and Certification Requirements

  • Insurance coverage, including Medicare, requires physician certification that the patient has a terminal illness with an estimated prognosis of 6 months or less if the disease runs its natural course, according to the American Gastroenterological Association 1, 2

Primary Care's Role in Hospice Referral

  • Primary care providers should provide "primary palliative care" and refer to hospice when patients have comfort-oriented goals with ≤6 months prognosis, as recommended by the American Gastroenterological Association 1, 3, 4
  • The American College of Physicians recommends that primary care providers provide timely referral to hospice for patients with comfort-oriented goals and prognosis ≤6 months, ideally in the context of preceding advance care planning and goals of care discussions 1, 2, 5

The Crisis of Late Referrals

  • The National Comprehensive Cancer Network recommends that hospice enrollment should occur earlier, as the median hospice length of stay is only 17-18 days, with 36% of patients dying within 7 days of admission 6
  • Hospice enrollment significantly reduces hospitalizations, ICU admissions, invasive procedures, and healthcare costs while improving both patient and caregiver quality of life, according to the National Comprehensive Cancer Network 6

Establishing Clear Referral Pathways

  • The American Gastroenterological Association recommends that healthcare providers work with local hospice teams to establish clear triggers and pathways for referral, with role clarity about "who does what" for primary palliative care versus specialist referral 1, 3, 4
  • The American College of Physicians recommends using objective triggers for hospice discussion, such as diagnosis of advanced disease, ICU admission, hospital admission, mechanical ventilation, or dialysis initiation 5
  • The National Comprehensive Cancer Network recommends scheduling a dedicated "hospice information visit" when prognosis reaches 6-12 months to ease the transition and ensure the physician remains involved 6

Hospice Care Duration and Referral Patterns

Current Statistics on Hospice Duration

  • The median length of stay for hospice patients is approximately 17-18 days, with the average (mean) being only 9 days, reflecting the persistent problem of late referrals that prevent patients from receiving the full benefit of hospice care, according to the NCCN Guidelines 7, 8
  • The median length of stay is 17.4 days in 2014, and the average (mean) length of stay is 9.1 days among Medicare beneficiaries with cancer (2010 data), as reported by the NCCN Guidelines 7, 8, 9
  • Approximately 36% of patients die or are discharged within 7 days of hospice admission, and only 43.3% of Medicare cancer patients accessed even 3 or more days of hospice care by 2007, highlighting the issue of late referrals 7, 8, 10

Benefits of Hospice Care

  • Hospice care is strongly associated with improved quality of life, reduced symptom burden, and better achievement of end-of-life wishes, but these benefits require adequate time in hospice to be realized, as emphasized by the NCCN Guidelines 8, 11, 12
  • Hospice enrollment leads to significantly lower hospitalization rates, fewer ICU admissions and invasive procedures, and important improvements in both patient and caregiver quality of life, according to the NCCN Guidelines 8, 11, 12, 13
  • Hospice care is also associated with lower healthcare costs in the last year of life, as reported by the NCCN Guidelines 8, 11, 12

Clinical Implications

  • The NCCN Guidelines recommend initiating hospice discussions immediately when a patient's functional status indicates a 6- to 12-month prognosis, rather than waiting for further decline 7, 8, 10
  • Key action points include scheduling a dedicated "hospice information" visit when prognosis reaches 6-12 months, and referring to hospice agencies as the first intervention for patients with months-to-weeks life expectancy, as recommended by the NCCN Guidelines 7, 8, 10, 14, 15
