Management of SLE-Associated Hearing Loss

Current Treatment Assessment

  • The American Academy of Otolaryngology-Head and Neck Surgery recommends completing the planned 3-week course of intratympanic dexamethasone and oral prednisolone, then reassessing with formal audiometry at 10-14 days from onset to determine if salvage therapy is needed 1
  • Patients with mild improvement at day 7, such as ability to hear snapping and fingernail flicking, reduced ear canal numbness, and decreased tinnitus, indicate partial response to therapy, with the greatest treatment benefit occurring within the first 2 weeks 1

Treatment Protocol

  • The standard treatment protocol involves twice-weekly dexamethasone for 2-3 weeks, with at least 4 injections over 2 weeks, as recommended by the American Academy of Otolaryngology-Head and Neck Surgery 3
  • Oral prednisolone dose of 60mg daily total meets the guideline-recommended 1 mg/kg/day for sudden sensorineural hearing loss 2

Next Steps and Timeline

  • Formal audiometry at day 10-14 will determine if adequate recovery has been achieved, defined as ≥10 dB improvement in pure tone average or ≥10% improvement in word recognition score 1
  • If hearing recovery is incomplete at 2-6 weeks, consider extended salvage intratympanic therapy with 4-6 additional injections of dexamethasone 5mg/mL over 2 weeks 1

Special Considerations for SLE

  • Patients with SLE history, such as cerebritis and class 3 nephritis, are at higher risk for steroid-related complications, particularly osteonecrosis and fractures 4
  • Intratympanic steroids minimize systemic exposure and are safe in patients with conditions requiring caution with systemic steroids, including SLE with prior organ involvement 5, 6

Salvage Options

  • Extended intratympanic steroids with 4-6 additional injections of dexamethasone 5mg/mL over 2 weeks shows 37-48% improvement rates in patients with incomplete initial recovery 3
  • Hyperbaric oxygen therapy combined with steroids may be considered within 1 month of onset, though this is not a strong recommendation 1

Monitoring for Complications

  • Monitor for hyperglycemia, especially in patients with high-dose oral prednisolone and SLE 6, 4
  • Watch for signs of SLE flare, though short-course high-dose steroids may actually help control it 4
  • Assess for tympanic membrane perforation, checking for persistent drainage or pain at injection sites 6
