Pancreatic Enzyme Replacement Therapy in Patients with Chronic Pancreatitis
Introduction to Pancreatic Enzyme Replacement
- The American Gastroenterological Association (AGA) recommends a starting dose of pancreatic enzyme replacement therapy (PERT) of at least 40,000 USP units of lipase per meal for a 70 kg adult patient 1, 2, 3
- The dose should be calculated based on the patient's weight, with a minimum of 500 units/kg/meal, but a higher dose of at least 40,000 USP units of lipase per meal is recommended 1, 2, 3, 4
Dosage Calculation and Administration
- For a 70 kg patient, the calculated dose is 70 kg x 500 units/kg = 35,000 units, but the guideline recommends starting with a minimum of 40,000 USP units of lipase per meal 1, 2, 3, 4
- The dose for snacks should be half of the meal dose, which is 20,000 USP units of lipase 1, 2, 3, 4
- Enzymes should be taken during meals, not before, and multiple capsules should be distributed throughout the meal 1, 2, 3, 5
Monitoring and Adjusting Therapy
- The response to therapy should be monitored by assessing the reduction of steatorrhea and improvement of gastrointestinal symptoms 1, 3
- The dose may need to be adjusted based on the patient's response, with a maximum dose of 2,500 units/kg/meal or 10,000 units/kg/day 4
- For a 70 kg patient, the maximum dose would be 175,000 units/meal or 700,000 units/day 4
Important Considerations
- Creon is an enteric-coated microsphere formulation that does not require acid-reducing agents, but these may already be used in most patients to enhance treatment efficacy 1, 2, 3
- Over-the-counter pancreatic enzyme preparations should not be used, as they are not standardized and may not be effective 1, 2
- Additional measures include routine monitoring of fat-soluble vitamin levels, recommending a low-to-medium fat diet with frequent small meals, and annual nutritional assessments 1, 2, 3
- DEXA scans should be performed every 1-2 years to assess osteoporosis risk 1, 4
