Guideline Recommendations for Switching from Nifedipine to Amlodipine

Dosing and Titration

  • Initiate amlodipine at 5 mg once daily when converting from nifedipine 10 mg; increase to 10 mg once daily only if systolic BP remains ≥140 mm Hg or diastolic BP ≥90 mm Hg after 2–4 weeks of therapy. This recommendation is based on a 2015 Canadian cardiology guideline. 1

Safety and Formulation Recommendations

  • The British Hypertension Society (1999) advises that immediate‑release nifedipine capsules should no longer be prescribed for hypertension management. 2
  • Immediate‑release nifedipine is contraindicated in patients with acute coronary syndromes unless they are receiving concurrent beta‑blocker therapy, as stated in a 2011 Circulation article. 4
  • For patients currently taking immediate‑release nifedipine 10 mg, a direct switch to amlodipine 5 mg once daily is strongly recommended to enhance safety, per the 1999 British Hypertension Society guidance. 2
  • Extended‑release nifedipine formulations (e.g., GITS, coat‑core) are acceptable alternatives but provide no clinical advantage over amlodipine, according to a 2024 European Heart Journal review. 5
