Diagnosis and Evaluation of Diabetes Insipidus

Urine Concentration and Osmolality

  • The normal concentrating range for urine osmolality is between 300-900 mOsm/kg, and a value of 498 mOsm/kg is well within this range, indicating normal concentrating ability 1
  • In diabetes insipidus, urine osmolality is characteristically <200 mOsm/kg despite elevated serum osmolality, which is the pathognomonic finding for this condition 2, 3

Serum Osmolality and Diagnostic Criteria

  • The critical diagnostic feature of DI is the combination of high serum osmolality (>300 mOsm/kg) with inappropriately low urine osmolality (<200 mOsm/kg) 2, 3
  • A serum osmolality of 301 mOsm/kg is only minimally elevated, suggesting mild dehydration from a 12-hour fast, and is not consistent with DI 4

Differential Diagnosis and Other Causes of Polyuria

  • Other causes of polyuria include diabetes mellitus, hypercalcemia, and medications, and should be considered as alternative explanations for symptoms 5
