Treatment Protocol for Herpetic Whitlow

First-Line Treatment Options

  • The Centers for Disease Control and Prevention recommends oral antiviral therapy with acyclovir, valacyclovir, or famciclovir as the first-line treatment for herpetic whitlow, which should be initiated as soon as possible after diagnosis 1
  • Acyclovir 400 mg orally three times a day for 7-10 days is a recommended treatment option for herpetic whitlow 1
  • Valacyclovir 1 g orally twice a day for 7-10 days is a recommended treatment option for herpetic whitlow 1
  • Famciclovir 250 mg orally three times a day for 7-10 days is a recommended treatment option for herpetic whitlow 1

Treatment Considerations

  • Treatment may be extended if healing is incomplete after 10 days of therapy, as recommended by the Centers for Disease Control and Prevention 1
  • Topical antiviral therapy is substantially less effective than systemic therapy and is not recommended, according to the Centers for Disease Control and Prevention 1 and other guidelines 2

Special Populations

Immunocompromised Patients

  • Higher doses of antiviral therapy may be required for immunocompromised patients, such as acyclovir 400 mg orally three to five times a day until clinical resolution, as recommended by the Centers for Disease Control and Prevention 3
  • For severe cases in immunocompromised patients, intravenous acyclovir 5 mg/kg every 8 hours may be required, according to the Centers for Disease Control and Prevention 3
  • If lesions persist during acyclovir treatment in immunocompromised patients, resistance should be suspected, and alternative treatments such as foscarnet 40 mg/kg IV every 8 hours until clinical resolution may be considered 3

Patient Education

  • Patients should avoid contact with the lesions to prevent autoinoculation or transmission, as advised by the Centers for Disease Control and Prevention 1

Follow-up Recommendations

  • Monitor for complete resolution of lesions, as recommended by the Praxis Medical Insights 2
  • If symptoms worsen despite treatment, consider secondary bacterial infection or resistance, and adjust treatment accordingly, as recommended by the Centers for Disease Control and Prevention 3

Treatment of Herpetic Whitlow in Special Populations

Immunocompromised Patients

  • For immunocompromised patients, the Centers for Disease Control and Prevention recommends higher doses of antiviral therapy, such as acyclovir 400 mg orally three to five times a day until clinical resolution, or intravenous acyclovir 5-10 mg/kg every 8 hours until clinical resolution 4

Recurrent Episodes

  • For patients with recurrent herpetic whitlow, the Centers for Disease Control and Prevention suggests episodic therapy, starting treatment at the first sign of prodrome or lesions, with options including acyclovir 400 mg orally three times a day for 5 days, or valacyclovir 500 mg orally twice a day for 5 days 4, 5
  • The Centers for Disease Control and Prevention also recommends suppressive therapy for frequent recurrences, with options including acyclovir 400 mg orally twice a day, or valacyclovir 250 mg orally twice a day, or valacyclovir 500-1000 mg orally once a day 4, 5

Long-Term Management

  • After 1 year of continuous suppressive therapy, the Centers for Disease Control and Prevention suggests considering discontinuation to assess the patient's rate of recurrent episodes 4, 5
