Management of Post-Bronchitis Cough in Smokers

Introduction to Guidelines

  • The American College of Chest Physicians recommends that smoking cessation must be the primary recommendation for patients with post-bronchitis cough, as 90% of patients will have complete resolution of their cough after quitting smoking 3, 4
  • Avoidance of respiratory irritants is the most effective means to improve or eliminate cough in chronic bronchitis, with a Grade A recommendation 3, 4

First-Line Treatment

  • The American College of Chest Physicians suggests that ipratropium bromide is the preferred first-line inhaler therapy for cough in chronic bronchitis, with a Grade A recommendation 3, 4, 5
  • Ipratropium bromide reduces cough frequency, cough severity, and sputum volume more reliably than short-acting β-agonists (SABAs) 5, 7

SABA Use

  • The American College of Chest Physicians recommends that short-acting β-agonists should be used to control bronchospasm and relieve dyspnea, with a Grade A recommendation 3, 4
  • SABAs may also reduce chronic cough in some patients, but show inconsistent results for cough improvement compared to ipratropium bromide 3, 4, 5

Treatment Algorithm

  • The treatment algorithm for post-bronchitis cough in smokers should start with smoking cessation counseling, followed by ipratropium bromide as first-line inhaler therapy, and then add a SABA if bronchospasm or dyspnea is present 3, 4, 5, 7, 8

Safety Concerns

  • The American College of Chest Physicians notes that paradoxical bronchospasm can occur with SABA use, which is rare but life-threatening, and requires immediate discontinuation of the medication 3, 4

Clinical Pitfalls to Avoid

  • The American College of Chest Physicians advises against prescribing a SABA without addressing smoking cessation, as this treats the symptom while ignoring the most effective cure 3, 4
  • The American College of Chest Physicians also recommends against using antibiotics for stable chronic bronchitis, as there is no role for long-term prophylactic antibiotics, with a Grade I recommendation 3, 4
