Gaucher Disease Treatment Guidelines

Introduction to Gaucher Disease Treatment

  • The American College of Medical Genetics and Genomics recommends enzyme replacement therapy (ERT) as the primary treatment for Gaucher disease, with Type 3 patients requiring immediate initiation and Type 1 patients requiring treatment when two or more disease manifestations are present 1, 2

Treatment Algorithm by Disease Type

  • For Type 1 Gaucher disease, the European Society of Human Genetics suggests initiating ERT when two or more of the following manifestations are present: thrombocytopenia, hepatosplenomegaly 1, 3
  • The American College of Medical Genetics and Genomics recommends substrate reduction therapy (SRT) as an alternative option for patients who cannot tolerate ERT or as combination therapy 1, 4
  • For Type 3 Gaucher disease, the American College of Medical Genetics and Genomics recommends immediate treatment initiation with ERT, and some evidence suggests combination therapy with ERT plus miglustat may stabilize or prevent neurological manifestations 1, 2, 3, 5
  • For Type 2 Gaucher disease, the American College of Medical Genetics and Genomics recommends only supportive care, as no currently effective disease-modifying treatment exists 1, 2, 3, 5

Pre-Treatment Evaluation Requirements

  • The American College of Medical Genetics and Genomics recommends completing the following assessments before initiating ERT: complete blood count, imaging for hepatosplenomegaly measurement, bone evaluation, neurological assessment for patients with Type 3 disease, Gaucher biomarker levels, and anti-GBA antibody levels 1, 2, 3

Monitoring During Treatment

  • The American Academy of Pediatrics recommends quarterly monitoring for infants and children to assess treatment response, development of new Gaucher manifestations, and need for additional interventions 1, 2

Emerging and Investigational Therapies

  • The National Institutes of Health reports that small molecule chaperone therapy for acid-glucosidase has completed Phase II trials with disappointing results 1, 5
