Management of Organic Delirium

First-line Management Approach for Delirium

  • The National Comprehensive Cancer Network recommends that benzodiazepines, including clonazepam, are not used as initial treatment for delirium in patients not already taking them 3
  • Benzodiazepines are sedating, have been identified as deliriogenic, and increase the risk of falls, according to the Annals of Oncology 1, 2
  • Current guidelines recommend first addressing reversible causes and using non-pharmacological interventions before considering medication, as stated by the Journal of the National Comprehensive Cancer Network 3

Appropriate Pharmacological Management of Delirium

  • For moderate delirium, the National Comprehensive Cancer Network recommends oral haloperidol, risperidone, olanzapine, or quetiapine 3, 4
  • For severe delirium with agitation, antipsychotic medications such as haloperidol, olanzapine, or chlorpromazine are recommended by the National Comprehensive Cancer Network 3, 4
  • Second-generation antipsychotics may offer benefit with fewer extrapyramidal side effects than first-generation agents, according to the Annals of Oncology 1, 5

Specific Circumstances When Benzodiazepines May Be Used in Delirium

  • Benzodiazepines are the treatment of choice as monotherapy for alcohol or benzodiazepine withdrawal delirium, as recommended by the Annals of Oncology 6, 7
  • Benzodiazepines may be added for agitation that is refractory to high doses of neuroleptics, according to the National Comprehensive Cancer Network 3, 4
  • Midazolam or lorazepam may be used as crisis intervention in delirium management for severe agitation and distress, as stated by the Annals of Oncology 6, 7

Risks and Cautions with Benzodiazepines in Delirium

  • Benzodiazepines increase the risk of falls, confusion, and respiratory depression, particularly in older or frail patients, according to the Annals of Oncology 6, 7
  • When benzodiazepines are used with antipsychotics, there is a risk of oversedation and respiratory depression, as warned by the Annals of Oncology 6

Clinical Decision-Making Algorithm

  • If pharmacological intervention is needed, the National Comprehensive Cancer Network recommends using antipsychotics for most cases of delirium 3, 4
  • For alcohol/benzodiazepine withdrawal delirium, the Annals of Oncology recommends using benzodiazepines 6, 7
  • For refractory agitation despite antipsychotics, the National Comprehensive Cancer Network suggests considering adding a benzodiazepine like lorazepam 3, 4

Important Caveats

  • Benzodiazepines should be used in the lowest effective dose and for the shortest time possible, as recommended by the Annals of Oncology 1, 2
  • Patients receiving benzodiazepines require close monitoring for excessive sedation, confusion, or respiratory depression, according to the Journal of the National Comprehensive Cancer Network 3
