Hypertension Management in Long COVID with Chronic Kidney Disease
Blood Pressure Target
- The American Heart Association suggests a target blood pressure of <130/80 mm Hg in patients with hypertension and CKD 1, 2, 4
- The KDIGO 2021 guideline recommends an even more aggressive target of systolic BP <120 mm Hg when tolerated, using standardized office BP measurement 3, 5
- For elderly patients, a slightly less aggressive target of <140/80 mm Hg may be appropriate 1, 4
First-Line Medication Selection
- ACE inhibitors or ARBs are the strongly recommended first-line agents for hypertension in CKD, particularly if albuminuria is present 1, 2, 3, 5, 6
- For severely increased albuminuria, ACE inhibitors or ARBs are a Class 1B recommendation 3, 5
- For moderately increased albuminuria, ACE inhibitors or ARBs are a Class 2C recommendation 3, 5
Monitoring Protocol
- Within 2-4 weeks of starting or increasing RAS inhibitor dose, check blood pressure, serum creatinine, and serum potassium 3, 5, 6
- Continue ACE inhibitor/ARB unless serum creatinine rises >30% within 4 weeks, symptomatic hypotension occurs, uncontrolled hyperkalemia develops, or eGFR <15 mL/min/1.73 m² with uremic symptoms 3, 4, 5, 6
Add-On Therapy Algorithm
- If BP remains uncontrolled on maximally tolerated RAS inhibitor, add a long-acting dihydropyridine calcium channel blocker as second-line therapy 1, 4, 6
- As third-line therapy, add a thiazide-like diuretic or loop diuretic if eGFR <30 mL/min/1.73 m² 1, 4, 6
Important Contraindications
- Never combine ACE inhibitor + ARB + direct renin inhibitor in CKD patients 3, 5, 6
Lifestyle Modifications
- Sodium restriction to <2 g/day improves BP control, especially with RAS inhibitors 3
- Physical activity should be encouraged but modified given the post-exertional malaise 3
