Polyethylene Glycol Administration in Adults with IBS-C

Standard Dosing Protocol

  • The American Gastroenterological Association recommends starting with 17 grams of PEG 3350 once daily, mixed in 4-8 ounces of liquid, for adults with IBS-C already on soluble fiber 1, 2

Efficacy Timeline and Duration

  • Response to PEG 3350 is durable over 6 months of continuous use in adults with IBS-C 1, 2

Combination with Existing Fiber Therapy

  • The American Gastroenterological Association and the American College of Gastroenterology guideline recommends continuing the current soluble fiber regimen while adding PEG 3350, as fiber can be used in combination with PEG 1, 2
  • Adequate hydration, such as 8-10 ounces of fluid with fiber supplements, is recommended when using PEG 3350 2

Dose Titration Strategy

  • If 17 grams once daily is insufficient after 1-2 weeks, the dose can be increased based on symptom response, with no clear maximum dose, allowing for individualized titration 3, 4

Expected Side Effects

  • Common side effects of PEG 3350 include abdominal distension, loose stool, flatulence, and nausea, which are generally mild to moderate and consistent with laxative therapy 1, 2, 3

Important Clinical Caveats

  • For patients with chronic kidney disease, PEG 3350 should only be used under direct physician supervision due to risk of fluid and electrolyte disturbances 5
  • The American Gastroenterological Association recommends avoiding the use of magnesium oxide as an alternative in patients with renal insufficiency due to hypermagnesemia risk 1, 5

Pain Management Consideration

  • If abdominal pain persists after optimizing PEG 3350 therapy, consider prescription secretagogues like lubiprostone, which may provide additional pain relief 4, 6
