Burn Management for Function-Sensitive Areas
Calculation and Classification
- The entire hand represents approximately 1% of total body surface area (TBSA) per the Lund-Browder method, which is the recommended standardized approach for measuring burned body surface area in both adults and children 1, 2, 3
- Burns on function-sensitive areas like hands are considered severe regardless of TBSA percentage, and deep burns in these areas meet criteria for specialized burn center management in adults and children 1, 2, 3
Clinical Significance and Management
- The American College of Surgeons recommends immediate referral to a burn specialist or burn center for hand burns involving partial-thickness or full-thickness depth, as these require specialized care beyond first aid to prevent functional disability 4
- Cooling the burn immediately with clean running water for 5-20 minutes is crucial to limit tissue damage and reduce pain, and this should be followed by loosely covering the burn with a clean, non-adherent dressing while arranging immediate transfer 4, 5
- Administering over-the-counter pain medications such as acetaminophen or NSAIDs is also recommended for pain management 4, 5
