Medical Necessity Assessment for Bilateral Lower Eyelid Entropion Repair
Indications for Surgery
- The American Academy of Ophthalmology recommends surgical repair of bilateral lower eyelid entropion when local measures fail to control symptoms such as eye pain or corneal irritation 1
- Surgery is immediately indicated if documentation shows failed conservative treatment, with persistent symptoms despite trial of artificial tears, lubricants, and/or temporary measures 1
- Objective corneal damage, such as superficial punctate keratitis, corneal abrasion, corneal ulceration, or corneal scarring, is also an indication for surgery 1
- Severe acute presentation, including corneal infiltrate, ulcer, or imminent perforation, requires urgent surgical intervention 2
Documentation Requirements
- Documentation of a trial period of conservative management with artificial tears and lubricating ointments is required before authorizing surgery 1
- Slit-lamp examination findings documenting presence or absence of corneal epithelial damage, punctate keratitis, or other corneal pathology are necessary 2, 1
- Clinical notes demonstrating persistent symptoms despite appropriate conservative measures are also required 1
