PRBC Infusion Guidelines for Pediatric Patients
Standard Infusion Parameters
- The American Academy of Pediatrics recommends that each PRBC unit must be completed within 4 hours of removal from temperature-controlled storage to minimize bacterial proliferation and hemolysis risk 1, 2
- For hemodynamically stable patients without active bleeding, the standard transfusion duration is 2-4 hours per unit 2
- The initial transfusion rate for pediatric patients is recommended to be 4-5 mL/kg/hour, which translates to approximately 132-165 mL/hour for a 33kg patient 2
Critical Monitoring Requirements
- Close vital sign monitoring is required during the first 30 minutes of transfusion to detect acute transfusion reactions 1, 4
- Clinical assessment must occur before, during, and after each unit transfused 1, 2
Special Considerations for Rate Adjustment
- The American Heart Association recommends slower transfusion rates for patients with cardiovascular comorbidities to reduce the risk of transfusion-associated circulatory overload (TACO) 1, 2
- For patients with reduced cardiac output, consideration should be given to rates at the lower end of the standard range 2
Practical Algorithm for Transfusion
- The transfusion protocol should include removing the unit from storage and beginning transfusion within 30 minutes, setting the initial rate at 4-5 mL/kg/hour, monitoring closely for the first 30 minutes, completing the transfusion within 2-4 hours, and reassessing clinically after each unit 1, 2, 4
