Enoxaparin Dosing Guidelines for Anticoagulation Therapy
Standard Prophylactic Dosing
- The American College of Cardiology recommends a standard dose of 40 mg subcutaneously once daily for venous thromboembolism prophylaxis in hospitalized medical patients with normal renal function 4, 1, 3
- For patients with severe renal impairment, the recommended dose is reduced to 30 mg subcutaneously once daily, as enoxaparin clearance decreases by 44% in severe renal impairment, leading to a 2-3 fold increased bleeding risk without dose adjustment 1, 2, 3
Standard Therapeutic Dosing
- The American Heart Association recommends a standard dose of 1 mg/kg subcutaneously every 12 hours for treatment of venous thromboembolism or acute coronary syndrome in patients with normal renal function 1, 3
- For patients with severe renal impairment, the recommended dose is reduced to 1 mg/kg subcutaneously once daily, as patients with CrCl <30 mL/min have 2.25 times higher odds of major bleeding with standard dosing 1, 2, 3
Critical Safety Considerations and Monitoring
- The American College of Chest Physicians recommends monitoring anti-Xa levels in all patients with CrCl <30 mL/min receiving prolonged treatment, with a target therapeutic anti-Xa range of 0.5-1.0 IU/mL for twice-daily dosing and >1.0 IU/mL for once-daily dosing 1, 2, 3
