Treatment of Tinea in Children

Diagnosis and Treatment Recommendations

  • The American Academy of Pediatrics recommends that children with tinea capitis should always receive systemic therapy, while localized cutaneous infections (corporis, cruris, pedis) can be treated with topical antifungals, reserving oral therapy for extensive disease or failure of topical treatment 1
  • The British Journal of Dermatology suggests obtaining mycological confirmation through microscopy with KOH and fungal culture before starting therapy when possible 1, 2
  • For tinea capitis, samples should be collected by scraping the scalp, plucking hairs, brush sampling, or swabbing 1, 2
  • Microscopy with potassium hydroxide preparation (10-30%) provides a rapid diagnosis, while culture on Sabouraud agar with cycloheximide for at least 2 weeks identifies the specific species 1, 2

Treatment Options

  • The American College of Dermatology recommends terbinafine as the preferred agent for Trichophyton infections (including T. tonsurans), with dosage based on weight 1
  • For Microsporum infections, griseofulvina is the preferred agent, with superior efficacy to terbinafine for these species 1, 3
  • Itraconazolo and fluconazolo are alternative agents with similar efficacy to griseofulvina but with shorter treatment durations 1

Supportive Care and Prevention

  • The British Journal of Dermatology recommends using antifungal shampoos as adjunctive therapy for tinea capitis to reduce the spread of spores 3, 4
  • General hygiene measures should be observed to control sources of infection or reinfection 3, 4
  • Screening of all family members and treatment of those found positive can prevent high rates of recurrence 3, 4

Monitoring and Follow-up

  • The endpoint for adequate treatment should be mycological cure, not just clinical response 1, 4
  • Repeat mycological sampling at the end of the standard treatment period and then monthly until mycological clearance is documented 1, 4
  • Continue treatment until mycological clearance is documented 1, 3
