Management of Anxiety in Patients Taking Alprazolam

Assessment and Treatment Strategy

  • For patients with inadequately controlled anxiety on their home alprazolam dose, address reversible causes first and consider adding lorazepam 0.5-1 mg orally up to four times daily as needed, according to the British Medical Journal guidelines 1, 2
  • The British Medical Journal recommends reducing lorazepam to 0.25-0.5 mg in elderly or debilitated patients, with a maximum of 2 mg in 24 hours 1, 2
  • Lorazepam is preferred over increasing alprazolam due to its intermediate duration, predictable pharmacokinetics, and ability to be dosed as needed, as suggested by the British Medical Journal 1, 2

Augmentation Strategy

  • For severe, persistent anxiety unresponsive to benzodiazepines alone, consider adding low-dose haloperidol 0.5-1 mg orally at night and every 2 hours when required, according to the British Medical Journal guidelines 1, 2
  • Aripiprazole can be increased from 5 mg to 10 mg daily if the patient is already on this medication for comorbid conditions, as recommended by Praxis Medical Insights 3

Medication Interactions

  • Avoid adding SSRIs or SNRIs if the patient is on other serotonergic agents due to serotonin syndrome risk, as advised by Praxis Medical Insights 3
