Treatment of Styes (Hordeolum)

First-Line Management

  • Incision and drainage is the primary treatment for purulent collections, as recommended by the Infectious Diseases Society of America 1

When Are Antibiotics Indicated?

  • Antibiotics should be considered only in specific circumstances, such as severe or extensive disease with multiple sites of infection, rapid progression with associated cellulitis, signs and symptoms of systemic illness, immunocompromised patients, or lack of response to conservative management, according to the Infectious Diseases Society of America 1

Antibiotic Selection (When Needed)

  • Topical antibiotic ointments (e.g., erythromycin, bacitracin) may be used for localized infection, as suggested by the American Academy of Ophthalmology 2
  • First-line oral options include Dicloxacillin (500 mg four times daily), Cephalexin/Keflex (500 mg four times daily), and Clindamycin (300-450 mg three times daily) for penicillin-allergic patients, as recommended by the Infectious Diseases Society of America 3
  • If MRSA is suspected, treatment options include Trimethoprim-sulfamethoxazole, Doxycycline (100 mg twice daily), and Clindamycin (300-450 mg three times daily), according to the Infectious Diseases Society of America 1, 3, 4

Role of Keflex (Cephalexin) and Augmentin (Amoxicillin-Clavulanate)

  • Keflex (Cephalexin) is generally reserved for more severe infections or when conservative management fails, as stated by the Infectious Diseases Society of America 3
  • Both Keflex (Cephalexin) and Augmentin (Amoxicillin-Clavulanate) are excessive for uncomplicated styes and may contribute to antibiotic resistance, according to the World Journal of Emergency Surgery 4

Duration of Treatment

  • If antibiotics are necessary, 5-7 days of therapy is typically sufficient, as recommended by the Infectious Diseases Society of America 3
  • Treatment should be individualized based on clinical response, according to the World Journal of Emergency Surgery 4

Special Considerations

  • Tetracyclines should not be used in children under 8 years of age, as stated by the Infectious Diseases Society of America 1
  • Contact lens wearers should discontinue contact lens use until the stye resolves, as recommended by the American Academy of Ophthalmology 2
