Management of Paraquat Poisoning After Initial Decontamination

Critical Assessment and Intervention

  • The patient has already vomited 6 times, indicating natural gastric emptying has occurred, and activated charcoal has been administered as appropriate initial decontamination, with stable vital signs but concerning SpO2 of 95% on room air, according to Pediatrics 1
  • Gastric lavage should NOT be performed at 6 hours post-ingestion, especially with documented caustic injury, as indicated by Circulation 4
  • Atropine is used for organophosphate/cholinergic poisoning, not paraquat, as stated by Pediatrics 1
  • The American Academy of Pediatrics recommends against the use of gastric lavage in patients with caustic injury, as supported by Pediatrics 1
  • The American Heart Association, as published in Circulation, advises against gastric lavage in cases of paraquat poisoning with caustic injury 4

Prognosis and Treatment

  • Urgent plasma paraquat concentration is the most important prognostic indicator, guiding intensity of treatment, and the presence of caustic burns generally indicates significant exposure, as noted in the context of Circulation 4
  • The American College of Medical Toxicology recommends urgent poison control consultation, which is essential for managing paraquat poisoning, as indicated by Circulation 4
  • Supportive care should include monitoring for development of acute kidney injury and avoiding supplemental oxygen unless absolutely necessary, as oxygen accelerates free radical formation in lungs, according to Pediatrics 1
