Semaglutide Formulations and Indications

Introduction to Semaglutide

  • Semaglutide is available under the brand names Ozempic, Wegovy, and Rybelsus, each with specific indications and formulations 1

Pharmacology and Mechanism of Action

  • Semaglutide, a GLP-1 receptor agonist, acts on the hypothalamus to suppress appetite, delay gastric emptying, increase glucose-dependent insulin release, and decrease glucagon secretion 2

Indications and Formulations

  • Ozempic and Rybelsus are primarily indicated for type 2 diabetes, while Wegovy is specifically approved for weight management, administered via weekly subcutaneous injection at a higher dose (2.4 mg) for obesity treatment 3

Dosage and Administration

  • The American Diabetes Association recommends Rybelsus in 3 mg, 7 mg, and 14 mg doses with gradual titration, and Ozempic as a weekly injection starting at 0.25 mg and titrating to 0.5-1.0 mg weekly 4
  • The starting dose of oral semaglutide is 3 mg once daily, with a first titration to 7 mg once daily after 30 days on the 3-mg dose, and a potential second titration to 14 mg once daily after 30 days on the 7-mg dose if additional glycemic control is needed, according to the Mayo Clinic Proceedings 5
  • Oral semaglutide should be taken on an empty stomach at least 30 minutes before first food, fluid, or other oral medications of the day, with no more than 120 mL (4 oz) of plain water only, as specified by the Mayo Clinic Proceedings 5
  • No dosage adjustments are required for patients with renal impairment, including those with eGFR ≥2 mL/min per 1.73 m², as stated by the Mayo Clinic Proceedings 5

Adverse Effects

  • Common adverse effects of semaglutide include gastrointestinal symptoms (nausea, vomiting, diarrhea), which typically improve with gradual dose titration 2
  • Assess for gastrointestinal side effects, particularly during dose escalation, as they are a common adverse effect of oral semaglutide, affecting 31-34% of patients 6, 7, 8

Cardiovascular Benefits

  • Semaglutide has demonstrated cardiovascular benefits in clinical trials, showing a 26% reduction in major adverse cardiovascular events in patients with established cardiovascular disease 9
  • Semaglutide reduces major adverse cardiovascular events by 26% (HR 0.74; 95% CI 0.58-0.95; P < 0.001) in patients with type 2 diabetes and established cardiovascular disease, as demonstrated in the SUSTAIN-6 trial 6, 7, 8
  • Semaglutide substantially reduced heart failure events or CV death (HR: 0.73; 95% CI: 0.62-0.87; P = 0.0005) in the FLOW trial (2024) 8

Weight Management

  • Oral semaglutide provides significant weight loss benefits compared to placebo, as reported by the American College of Clinical Endocrinologists 10, 6
  • The American Association of Clinical Endocrinologists-aligned data suggests that GLP-1 receptor agonists are recommended as preferred agents for weight management in patients with diabetes and obesity 10, 6, 11
  • The American Diabetes Association recommends semaglutide as a preferred agent for weight management in patients with diabetes and obesity, with a focus on cardiovascular risk reduction 12

Special Considerations

  • For patients observing Ramadan fasting, experts recommend titrating the dose to at least 7 mg approximately 2-3 weeks before Ramadan to ensure therapeutic efficacy and patient tolerance, according to Diabetes/Metabolism Research and Reviews 13
  • During Ramadan, dosing should be taken at Iftaar (breaking of fast) to maintain proper absorption, as recommended by Diabetes/Metabolism Research and Reviews 13

Monitoring and Evaluation

  • Monitor glycemic response throughout the titration period, and evaluate renal function before and during treatment 8
  • Evaluate the patient's cardiovascular and renal status before initiating semaglutide treatment, and monitor the patient's glycemic response and potential side effects 8

Recommendations and Approvals

  • The FDA has approved semaglutide as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes, and to reduce the risk of major adverse cardiovascular events in adults with type 2 diabetes and established cardiovascular disease 6, 11
  • GLP-1 receptor agonists, including oral semaglutide, are recommended for patients with established cardiovascular disease due to their demonstrated cardiovascular benefits, according to the American College of Cardiology 10, 6, 11
