Cardiovascular Benefits of Empagliflozin in Patients with Type 2 Diabetes

Introduction to Empagliflozin Benefits

  • The American College of Cardiology recommends that patients with type 2 diabetes and clinical atherosclerotic cardiovascular disease should have an SGLT2 inhibitor with proven cardiovascular benefit added to their treatment regimen, even when glycemic control is adequate 1, 2
  • Baseline A1C does not modify the cardiovascular benefits of SGLT2 inhibitors, with secondary analyses from multiple studies demonstrating consistent risk reduction regardless of whether patients have well-controlled diabetes 1, 2

Cardiovascular Risk Reduction

  • Empagliflozin reduced cardiovascular death by 38% (HR 0.62, 95% CI 0.49-0.77) and all-cause mortality by 32% (HR 0.68, 95% CI 0.57-0.82) in patients with established atherosclerotic cardiovascular disease 3
  • The European Society of Cardiology recommends SGLT2 inhibitors with proven cardiovascular benefit in patients with type 2 diabetes and peripheral arterial disease specifically for mortality reduction 4

Combination Therapy with GLP-1 RA

  • The American College of Cardiology explicitly states that it is reasonable to use both an SGLT2 inhibitor and a GLP-1RA with demonstrated cardiovascular benefit concomitantly, even though combination therapy has not been specifically studied for cardiovascular disease risk reduction 1, 2
  • DURATION-8 demonstrated that nonglycemic effects (blood pressure and weight reduction) may be additive when combining these drug classes 5, 1

Practical Considerations

  • The American College of Cardiology recommends starting empagliflozin 10 mg daily, which has equivalent cardiovascular benefit to the 25 mg dose 3
  • If eGFR is 30-44 mL/min/1.73 m², use 10 mg daily; contraindicated if eGFR <30 mL/min/1.73 m² 6
  • Assess for volume depletion risk, particularly given hypertension treatment and potential diuretic use 7
  • Educate about genital mycotic infection risk and importance of hygiene 6
  • Monitor for euglycemic diabetic ketoacidosis symptoms, though risk is primarily in type 1 diabetes 8
  • Adjust Mounjaro dose if hypoglycemia occurs, though risk is low without insulin or sulfonylureas 1

Specific Patient Considerations

  • Given severe peripheral artery disease, empagliflozin is preferred over canagliflozin, as the American College of Cardiology recommends avoiding canagliflozin in patients with severe PAD, prior amputations, or peripheral neuropathy due to amputation risk in CANVAS 5, 2
  • Empagliflozin showed no increased amputation risk in EMPA-REG OUTCOME, even in the peripheral artery disease subgroup 4
