Amisulpiride Dosing Guidelines

Dosing Recommendations

  • The American College of Psychiatry recommends starting amisulpiride at 400-800 mg/day for acute exacerbations of schizophrenia with predominantly positive symptoms, with evidence supporting initiation at 800 mg/day for maximal efficacy without significant additional side effects 1
  • The 2025 INTEGRATE guidelines from the American Psychiatric Association recommend amisulpride as a second-line treatment option at standard antipsychotic doses 1
  • For patients with primary negative symptoms, the American Psychiatric Association recommends starting at 50-100 mg/day, with doses of 50 mg twice daily specifically recommended for predominant negative symptoms where positive symptoms are not a concern 2, 3

Administration and Titration

  • The American Psychiatric Association recommends that amisulpride can be initiated at the target therapeutic dose without gradual titration, with the 800 mg/day starting dose given from day one with low risk of extrapyramidal symptoms 2, 3
  • Twice-daily dosing (e.g., 50 mg BID) is commonly used for negative symptoms, as recommended by the American Psychiatric Association 2, 3

Amisulpiride Treatment Guidelines

Dose Selection and Administration

  • The American Psychiatric Association recommends maintaining a therapeutic dose of amisulpiride for at least 6 weeks before determining treatment response, as most non-responders within the first 6 weeks will not respond at later time points 4
  • The American Psychiatric Association suggests that a duration of adequate trial of at least 6 weeks is necessary to assess treatment response, with most non-responders not responding at later time points 4
