Essential Tremor Diagnostic Criteria and Management
Clinical Features and Diagnosis
- Consciousness remains intact during tremor episodes, and patients may experience a good response to low-dose voltage-gated sodium channel blockers, especially carbamazepine/oxcarbazepine, with a typical tremor frequency ranging between 4-8 Hz 1
- Tremor may worsen with emotional stress, caffeine consumption, and physical exertion, and can be differentiated from other types of tremor, such as enhanced physiologic tremor, which is usually temporary and related to specific triggers 2
- The American Academy of Neurology recommends considering the duration of attacks, age of onset, and presence of other neurologic or systemic problems when diagnosing essential tremor, with red flags including duration of attacks greater than 1 minute, age of onset over 20 years, and abnormalities in brain CT/MRI scanning 1
Differential Diagnosis and Exclusionary Criteria
- Essential tremor must be differentiated from other types of tremor, including Parkinson's disease tremor, dystonic tremor, and cerebellar tremor, with the International Parkinson and Movement Disorder Society recommending a careful evaluation of motor and non-motor features 3
- The following conditions must be excluded: cerebrovascular disease, demyelinating disease, metabolic disorders, brain trauma, and psychological disorders, according to the American Academy of Neurology 1
Severity Classification and Management
- Severity is typically classified based on functional impact, with the American Academy of Neurology recommending the following classification: mild (minimal interference with daily activities), moderate (some interference with daily activities), and severe (significant interference with daily activities) 4
- Clinicians can accurately diagnose essential tremor and initiate appropriate treatment when symptoms interfere with function or quality of life, with the European Federation of Neurological Societies recommending a multidisciplinary approach to management 5
