Teicoplanin Dosing Guidelines for Adults
Introduction to Teicoplanin Dosing
- The American College of Cardiology recommends a loading dose of 12 mg/kg every 12 hours for three doses, followed by a maintenance dose of 6-12 mg/kg once daily, with higher doses recommended for severe infections to achieve target trough concentrations of at least 20 mg/L 1
Standard Dosing Regimen
- A standard dosing regimen consists of a loading dose of 12 mg/kg every 12 hours for three doses, followed by a maintenance dose of 6-12 mg/kg once daily, with dose selection based on infection severity 1, 2
- For severe infections, such as endocarditis, septic arthritis, osteomyelitis, and complicated bacteremia, a higher maintenance dose of 12 mg/kg daily is recommended 1
- For high-risk MRSA infections, a dose of 6-12 mg/kg IV every 12 hours for 3 doses, then 6-12 mg/kg IV once daily is recommended 3, 4
Dosing Adjustments Based on Renal Function
- For patients with normal renal function, a dose of 6-12 mg/kg every 24 hours is recommended 2
- For patients with moderate renal impairment, a dose of 6-12 mg/kg every 48 hours is recommended 2
- For patients with severe renal impairment, a dose of 6-12 mg/kg every 72 hours is recommended 2
- For hemodialysis patients, a loading dose of 12 mg/kg, followed by 6 mg/kg on days 2 and 3, and maintenance of 6 mg/kg once weekly is recommended 2
Target Serum Concentrations
- For severe infections, such as endocarditis, septic arthritis, and osteomyelitis, a target trough concentration of at least 20 mg/L is recommended 1, 2
- Therapeutic drug monitoring is recommended for severe infections to ensure adequate dosing 1, 2
Special Situations
- For MRSA pneumonia, higher doses of 12 mg/kg should be considered when MIC values of MRSA to glycopeptides are relatively high 3
- For critically ill patients with increased volume of distribution, such as sepsis, a higher loading dose regimen is recommended 1
- For bone and joint infections, a higher maintenance dose of 12 mg/kg daily is recommended 1
Safety Considerations
- Regular monitoring of renal function is recommended, especially in patients receiving prolonged therapy or concomitant nephrotoxic medications 2
