Nystatin Treatment Guidelines

Standard Dosing Regimens

  • The standard dosing regimen for nystatin suspension in adults with oropharyngeal candidiasis is 4-6 mL (400,000-600,000 units) four times daily, continued for 7-14 days, as recommended by the Infectious Diseases Society of America 1, 2
  • The American College of Physicians recommends a dose of 1-2 pastilles (200,000 units each) four times daily for nystatin pastilles, with a duration of 7-14 days 1, 2

Clinical Context and Effectiveness

  • The Infectious Diseases Society of America recommends nystatin as first-line therapy for mild oropharyngeal candidiasis, alongside clotrimazole 1
  • For moderate to severe oropharyngeal candidiasis, the American College of Physicians recommends oral fluconazole 100-200 mg daily for 7-14 days, which is superior to nystatin 1, 2

Special Situations

  • The Centers for Disease Control and Prevention recommends standard nystatin dosing (4-6 mL four times daily or 1-2 pastilles four times daily) for 7-14 days for denture-related candidiasis, in addition to mandatory disinfection of the denture to prevent reinfection 1, 2, 3
  • The American College of Obstetricians and Gynecologists recommends nystatin 100,000-unit vaginal tablets once daily for 14 days for vaginal candidiasis 2, 3

Common Pitfalls

  • The Infectious Diseases Society of America notes that nystatin is generally less effective than fluconazole for moderate-to-severe or systemic infections 3
  • Clinical response to nystatin therapy should be evident within 48-72 hours; if not, consider alternative diagnosis or resistant organisms, as recommended by the American College of Physicians 3
