DAPT for Ectatic Left Circumflex Coronary Artery

Primary Recommendation

  • The European Society of Cardiology recommends initiating ticagrelor (180 mg loading dose, then 90 mg twice daily) plus aspirin (75-100 mg daily) for 12 months in patients with acute coronary syndrome, regardless of whether PCI is performed 1, 2, 3
  • For stable coronary artery disease requiring PCI, the European Society of Cardiology recommends using clopidogrel (600 mg loading dose, then 75 mg daily) plus aspirin (75-100 mg daily) for 12 months after stent implantation 1, 2, 3

Clinical Context Determines DAPT Strategy

  • The European Society of Cardiology recommends single antiplatelet therapy with either aspirin alone (75-100 mg daily) or clopidogrel alone (75 mg daily) for stable CAD patients not undergoing revascularization 1, 2
  • Ticagrelor is the first-line P2Y12 inhibitor for all ACS patients, including those with anatomical variants like coronary ectasia 1, 2, 3

Key Considerations for Coronary Ectasia

  • The European Society of Cardiology states that coronary ectasia does not change standard DAPT recommendations, as guidelines are based on clinical presentation (ACS vs stable CAD) and whether revascularization is performed, not on anatomical variants 1, 2, 3
  • Focus on the clinical syndrome (ACS vs stable) rather than the anatomical finding when selecting DAPT 1, 2, 3

Bleeding Risk Mitigation

  • The European Society of Cardiology recommends implementing measures to minimize bleeding complications while on DAPT, including using radial (not femoral) arterial access if PCI is performed, maintaining aspirin dose at 75-100 mg daily, and prescribing a proton pump inhibitor (PPI) with DAPT to reduce gastrointestinal bleeding 1, 2, 3

Duration Modifications

  • The European Society of Cardiology recommends shortening DAPT to 6 months (or less) if the patient has excessive bleeding risk (PRECISE-DAPT score ≥25), then continuing single antiplatelet therapy 1, 2, 3
  • After the initial DAPT period, transition to single antiplatelet therapy for long-term cardiovascular protection 1, 2

Common Pitfalls to Avoid

  • The European Society of Cardiology recommends avoiding the use of prasugrel in patients with prior stroke or TIA (contraindicated) 1, 2, 3
  • The European Society of Cardiology recommends not discontinuing DAPT within the first month after stent placement for elective surgery 1, 2, 3
  • The European Society of Cardiology recommends not assuming coronary ectasia requires different DAPT than standard coronary disease 1, 2, 3
