Alendronate Therapy Guidelines

Essential Supplementation and Dosing

  • The American College of Oncology recommends that all patients taking alendronate must take supplemental calcium (1,000-1,200 mg daily) and vitamin D (400-800 IU daily) if dietary intake is inadequate 1
  • The weekly dosing schedule for alendronate is one 70 mg tablet once weekly on the same chosen day each week, as recommended by the Journal of Clinical Oncology 1
  • The daily dosing alternative for alendronate is one 5 mg or 10 mg tablet once daily every morning, according to the Journal of Clinical Oncology 1

Lifestyle Modifications for Bone Health

  • Regular weight-bearing exercise is recommended for bone health optimization, as suggested by the Journal of Clinical Oncology 1
  • Smoking cessation is advised, as recommended by the Journal of Clinical Oncology 1
  • Limiting alcohol consumption to two or fewer units daily is recommended, according to the Journal of Clinical Oncology 1
  • Maintaining adequate calcium intake through diet and supplements is essential, as recommended by the Journal of Clinical Oncology 1

Rare but Serious Long-Term Complications

  • The Annals of Oncology reports that osteonecrosis of the jaw (ONJ) is a rare but serious complication, with risk increasing after 2 years of therapy, and recommends maintaining good dental hygiene and informing your dentist you are taking bisphosphonates 2

Guidelines for Taking Oral Fosamax (Alendronate) to Minimize Adverse Effects

Important Precautions

  • The American Academy of Family Physicians recommends that patients with abnormalities of the esophagus or those who cannot sit or stand upright for at least 30 minutes should not take alendronate 3
  • Patients at increased risk of aspiration should not receive alendronate solution, as recommended by the American Academy of Family Physicians 3

Additional Lifestyle Recommendations

  • The American Society of Clinical Oncology recommends that patients engage in weight-bearing exercise, stop smoking, limit alcohol consumption to two or fewer units daily, and maintain adequate calcium intake through diet and supplements 4, 5

Monitoring

  • The American College of Rheumatology recommends that for patients on long-term bisphosphonate therapy, bone mineral density (BMD) with vertebral fracture assessment or spinal x-ray should be performed every 1-2 years 6, 7
  • Some experts, including those from the Infectious Diseases Society of America, recommend considering interruption of bisphosphonate therapy after five years due to concerns about long-term safety 8

Guidelines for Coordinating Levothyroxine and Alendronate Therapy

Alendronate Administration

  • Alendronate should be taken on an empty stomach with a full glass of water (6–8 oz), at least 30 minutes before any food or beverage is consumed. 9
  • After taking alendronate, patients must remain upright (sitting or standing) for at least 30 minutes and should not lie down until after eating the first food of the day. 9

Levothyroxine and Alendronate Timing

  • Levothyroxine should be taken first thing in the morning on an empty stomach; alendronate must be taken at least 4 hours later to ensure levothyroxine absorption before introducing other medications. 9
  • An alternative schedule is to take levothyroxine in the morning and alendronate the following morning before breakfast, which simplifies weekly dosing of the 70 mg alendronate formulation. 9

General Absorption Requirements

  • Both levothyroxine and alendronate require strict empty‑stomach conditions for optimal absorption. 9
