Hydrocele Diagnosis and Management
Clinical Presentation and Diagnosis
- Complete transillumination indicates a clear fluid collection, which is characteristic of hydrocele, as stated by the American College of Radiology 1, 2, 3
- A bowel-containing inguinal hernia would not demonstrate complete transillumination, as bowel contents are solid/gas-filled structures that do not allow uniform light transmission, according to the American College of Radiology 1, 2
- Varicocele presents as a "bag of worms" feeling on palpation and does not transilluminate uniformly, as varicoceles are dilated veins containing blood, which blocks light, as noted by the American College of Radiology 1, 2
Differential Diagnosis
- Hematocele would not transilluminate because blood is opaque and blocks light transmission, with blood-filled collections appearing dark on transillumination 1, 2
- Testicular tumor presents as a solid mass that does not transilluminate, as solid tissue blocks light transmission entirely, although hydroceles can occasionally mask underlying testicular tumors, particularly in young men 1, 2
