GLP-1 Receptor Functions and Clinical Implications

Primary Functions of GLP-1 Receptors

  • GLP-1 receptors mediate glucose-dependent insulin secretion, ensuring that insulin is only stimulated when blood glucose levels are elevated, which explains the low risk of hypoglycemia with GLP-1 receptor agonists 1
  • GLP-1 receptors inhibit glucagon secretion from pancreatic alpha cells in a glucose-dependent manner, reducing hepatic glucose production 1
  • They delay gastric emptying by inhibiting gastric peristalsis while increasing pyloric tone, which contributes to postprandial glucose control 1, 2

Anatomical Distribution of GLP-1 Receptors

  • GLP-1 receptors are primarily located in the sinoatrial node of the heart, hypothalamus and brainstem nuclei, hippocampus, neocortex, spinal cord, cerebellum, arterial walls, and myenteric plexus of the gastrointestinal tract 1, 2, 3

Metabolic and Gastrointestinal Effects

  • GLP-1 receptor activation leads to reduced phasic gastric contractions, delayed gastric emptying, reduced gastric acid secretion, and increased fasting and postprandial gastric volumes 1, 2
  • The motility effects are more pronounced in individuals with normal or rapid baseline gastric emptying 2
  • Short-acting GLP-1 receptor agonists have more pronounced effects on delaying gastric emptying than long-acting formulations due to tachyphylaxis 1, 2

Central Nervous System Effects

  • GLP-1 receptors in the hypothalamus and brainstem nuclei mediate appetite, satiety, energy intake, and expenditure 1, 3
  • Systemic GLP-1 receptor agonists suppress appetite and induce meal termination by reaching the arcuate nucleus in the hypothalamus and the area postrema and nucleus tractus solitarius in the medulla 3

Cardiovascular Effects

  • Cardioprotective effects of GLP-1 receptor activation include improved myocardial substrate utilization, anti-inflammatory and anti-atherosclerotic effects, reduced myocardial ischemia injury, and lower systemic and pulmonary vascular resistance 3

Pancreatic Islet Effects Beyond Insulin Secretion

  • GLP-1 receptor activation may promote beta cell proliferation and differentiation while protecting against apoptosis, potentially preserving pancreatic beta cell mass 1

Clinical Implications

  • The glucose-dependent nature of GLP-1 receptor activation explains the low risk of hypoglycemia with GLP-1 receptor agonist therapies 1
  • The delay in gastric emptying contributes significantly to increased satiety, weight loss, and glucose-lowering effects 1, 2
  • Caution should be used in patients with pre-existing gastroparesis or delayed gastric emptying, as GLP-1 receptor activation may exacerbate symptoms 2
