Pneumocystis Jirovecii Pneumonia Prophylaxis in Children

Dosing and Administration

  • For a 10kg child, the recommended dose of Septran DS (trimethoprim-sulfamethoxazole) for Pneumocystis jirovecii pneumonia prophylaxis is approximately one-half tablet of Septran DS daily, or one-half tablet three times weekly, according to the Clinical Infectious Diseases guidelines 1
  • The American Academy of Pediatrics recommends ensuring adequate hydration during treatment to prevent crystalluria and stone formation, as suggested by Clinical Infectious Diseases 1

Monitoring and Precautions

  • Regular monitoring for adverse effects, including complete blood counts with differential and platelet counts, is essential, as recommended by Praxis Medical Insights 2
  • Renal function tests, especially in patients with underlying kidney disease, should be performed, as suggested by Clinical Infectious Diseases 1
  • For patients with creatinine clearance 15-30 mL/min, the dose should be reduced by half, according to Clinical Infectious Diseases 1

Alternative Regimens

  • If TMP-SMX is not tolerated, alternative prophylactic regimens include Dapsone (2 mg/kg/day, maximum 100 mg daily), as recommended by Praxis Medical Insights 2

Duration of Prophylaxis

  • Prophylaxis should be continued as long as the immunosuppressive condition persists, as recommended by MMWR Recommendations and Reports and Praxis Medical Insights 3, 2
