Ertapenem Dosing for Contaminated Colon Surgery

Indications and Rationale

  • Ertapenem 1 g once daily is specifically recommended for patients with inadequate/delayed source control or those at high risk of infection with community-acquired ESBL-producing Enterobacterales in intra-abdominal infections 1, 2

Administration Guidelines

  • For treatment of established infection, continue therapy for 4 days in immunocompetent and non-critically ill patients if source control is adequate 1, 3
  • For immunocompromised or critically ill patients, extend therapy up to 7 days based on clinical condition and inflammatory markers 1, 3

Special Considerations

  • For patients in septic shock, consider alternative carbapenems with more frequent dosing, such as Meropenem 1 g q6h by extended infusion or continuous infusion 1
  • For patients in septic shock, consider alternative carbapenems with more frequent dosing, such as Doripenem 500 mg q8h by extended infusion or continuous infusion 1
  • For patients in septic shock, consider alternative carbapenems with more frequent dosing, such as Imipenem/cilastatin 500 mg q6h by extended infusion 1, 2, 4

Reconstitution and Administration of Ertapenem

Reconstitution Process

  • Ertapenem 1g powder contributes approximately 1ml to the final volume when reconstituted 5
  • Adding 3.2ml of diluent to the powder results in a total volume of 4.2ml 5

Clinical Applications

  • Treatment of intra-abdominal infections at a dose of 1g once daily 6

Ertapenem Reconstitution and Administration

Stability and Storage

  • Carbapenems, including ertapenem, are particularly unstable after reconstitution and should not be stored for extended periods 7
