Treatment of Acute Necrotizing Encephalopathy

Immediate Recognition and Stabilization

  • Transfer patients immediately to an ICU setting with neurological specialist availability within 24 hours, and position them with head elevated at 30 degrees to reduce intracranial pressure, as recommended by Praxis Medical Insights 1
  • Intubate patients for airway protection if they progress to Grade III-IV encephalopathy, and obtain urgent neuroimaging to confirm bilateral thalamic lesions and exclude hemorrhage 1
  • Avoid sedation when possible, and use small doses of short-acting benzodiazepines or propofol if required for severe agitation 1
  • Ensure enteral nutrition when possible, or total parenteral nutrition if enteral route unavailable, and monitor for and promptly treat secondary infections 1

Antiviral Therapy for Influenza-Associated Cases

  • Administer oseltamivir or other neuraminidase inhibitors immediately for influenza-associated ANE, as recommended by the Journal of Infection and Thorax 2 3

Critical Pitfalls to Avoid

  • Avoid aspirin in children due to Reye's syndrome risk, as recommended by Thorax 3
  • Do not rely on cerebrospinal fluid analysis to guide treatment, as CSF may be normal or show only mild protein elevation 3
